Early Childhood CARES: Lane County child referral: practical steps
UO Early Childhood CARES · Lane counties
Lane County EI/ECSE child-referral form with English and Spanish phone routes, family-consent instructions, and an explanation of intake follow-up.
Where and how to proceed: prepare, file and follow up
Start here: Confirm the child lives in Lane County and is birth to five with developmental concerns. Discuss screening results with the family before any third-party referral.
1. Is this for me?
Any child in Lane County birth to five with developmental concerns may be referred. Parents/guardians can refer their own child; other people must have family consent. Intake/evaluation determines eligibility.
Service area: Lane counties
2. What should I prepare?
Some documents may be conditional. Ask about alternatives. Checkmarks are temporary and are not saved or sent.
3. Where and how do I submit?
Use the child-referral form, call 541-346-2578 (English) or 541-346-0742 (Spanish), or email ecreferrals@uoregon.edu. Review the Ages and Stages Questionnaire and discuss concerns with the family before a third-party referral.
- Confirm the child lives in Lane County and is birth to five with developmental concerns. Discuss screening results with the family before any third-party referral.
- With family consent, submit the child-referral form, email ecreferrals@uoregon.edu or call 541-346-2578 (English) / 541-346-0742 (Spanish).
- The intake coordinator contacts the parent/guardian to review information and explain next steps. Decide with the family whether to continue; every enrollment step is optional.
Timing: Contact the intake team for current evaluation scheduling and next steps. A referral does not establish eligibility or promise a service start date.
4. What will it cost?
Early Childhood CARES publishes eligible EI/ECSE services as free, with no insurance required. Referral does not guarantee eligibility or a particular service.
5. What happens after I submit?
Ask intake to confirm receipt and explain screening/evaluation scheduling. Keep contact notes and any appointment/consent information; no response deadline is established here.
6. What if something goes wrong?
Never refer a child without family consent. Families may decline any enrollment step. If the child is outside Lane or at the school-age transition, ask for the correct county/district route.
Language, disability and offline help
Use the published Spanish phone 541-346-0742 or English phone 541-346-2578 and ask about communication access and alternative form formats.
Choose your step and follow-up instructions
Work through the existing instructions for this route. Reviewing a step does not complete a legal requirement.
Never refer without family consent. The program says each enrollment step is optional and the family can decline. Eligible EI/ECSE services are free and do not require insurance; a referral is not approval.
