Children birth to five before entering kindergarten in Crook, Deschutes or Jefferson County who may have developmental delays. Anyone may make a referral; screening/evaluation determines service eligibility.

3. Where and how do I submit?
Use the online referral form or the residence/age-specific contact: Deschutes/Crook birth-two 541-312-1947, ages three-five 541-312-1971; Jefferson birth-five 541-546-6841. Bilingual intake contact: 541-312-1949.

- Discuss screening with intake. If evaluation is offered and the child is eligible, the family and team develop an Individual Family Service Plan (IFSP). Confirm the actual appointment location.

4. What will it cost?
The Central Oregon program publishes no-cost EI/ECSE services. Screening/referral is not automatic eligibility or a reserved appointment.

Confirm receipt and the appropriate age/county team. Ask whether screening or evaluation is next and confirm its time, place and family participation.
6. What if something goes wrong?
Do not use the Central Oregon contacts as direct intake for every county in the larger High Desert regional-grantee map. Ask about the correct partner outside Crook/Deschutes/Jefferson.
